WHERE TO FILE:     CITY HALL, COMMUNITY ROOM “A”

ANY SENIOR CITIZEN INGLEWOOD RESIDENT THAT IS 62 YEARS OF AGE OR OLDER PRIOR TO JULY 1, 2015, WITH AN INCOME OF LESS THAN $17,700 PER YEAR, PER HOUSEHOLD, IS ELIGIBLE TO FILE A CLAIM FOR REFUND OF THE UTILITY USERS TAXES PAID DURING THE PERIOD JULY 1, 2015 THROUGH JUNE 30, 2016 IN THE NAME OF THE APPLICANT(S).

CLAIM FORMS MAY BE PICKED UP AT CITY HALL, FINANCE DEPT. CUSTOMER SERVICE. COMPLETED FORMS MUST BE SUBMITTED WITH THE FOLLOWING DOCUMENTATION; BIRTHDATE VERIFICATION, SOCIAL SECURITY NUMBER, INCOME FROM ALL SOURCES IN THE HOUSEHOLD, UTILITY BILLS SHOWING THE TAX PAID AND ANY OTHER INFORMATION TO SUPPORT YOUR CLAIM.

SENIOR CITIZENS - PLEASE COME TO THE CITY HALL, COMMUNITY ROOM “A”, 1ST FLOOR ON THE FOLLOWING DATES BETWEEN THE HOURS OF 9:00 A.M TO 12:00 P.M. WITH LAST APPLICANT @ 11:30 A.M. TO OBTAIN REFUND IN PERSON:

JULY 19, 20, 22, 25, 26, 29

AUGUST 1, 2, 8, 9, 10, 11, 16, 17, 18

DUE TO STAFFING AVAILABILITY WE WILL ONLY BE ABLE TO ASSIST 20 APPLICANTS A DAY AND ALL APPLICANTS ARE TAKEN ON A FIRST COME FIRST SERVE BASIS.

IF YOU ARE UNABLE TO ATTEND ONE OF THESE DATES YOU CAN SUBMIT A COMPLETED CLAIM FORM WITH THE ABOVE REQUIRED INFORMATION TO THE FINANCE DEPARTMENT, CUSTOMER SERVICE AFTER AUGUST 18, 2016 AND A CHECK WILL BE MAILED.
